Ribbon-cutting marks grand-opening of the new building for cardiovascular doctors in Grand Chute

APPLETON, Wis. (Apr. 3, 2019) – On Monday, April 1 a ceremonial ribbon-cutting for the staff of the new Heart and Vascular Institute of Wisconsin marked the grand opening of the practice, located at 5045 W. Grande Market Drive in Appleton.

Fifteen physicians and 60 staff members from the cardiovascular team at ThedaCare left the hospital to form a new independent practice and put the focus back on patient care. This marks the start of a new chapter and a new way for these physicians to serve the community.

“We’re so excited to be able to still serve the patients we know and love, just in a new facility with state-of-the-art equipment,” Dr. Carrie Chapman, cardiologist, said. “We’re proud to not only be offering care that is so technologically advanced, but it’s also affordable. We have preventative care services that start at $25.”
The official public open house is on Saturday, April 27 from 10 a.m. to 2 p.m. This will be the first chance the public will have to officially tour the building, meet the physicians, and see what the new Heart and Vascular Institute of Wisconsin has to offer.

Providing care in Appleton and surrounding communities since 1968, group opens Heart and Vascular Institute of Wisconsin

APPLETON, Wis. (FEB. 14, 2019) – The physicians currently providing cardiovascular care at ThedaCare will form a new independent practice on April 1, called the Heart and Vascular Institute of Wisconsin. This group will begin serving patients in early April at their new location at 5045 W. Grande Market Drive in Appleton, Wisconsin, just off College Avenue, near the Appleton International Airport. Additional regional office locations in or near Oshkosh, New London, Waupaca, Shawano, and Chilton will soon be announced and will bring the same quality care to patients in those areas.

“It is our responsibility to let current patients know of this change to ensure safety through continuity and access of care,” Dr. Carrie Chapman, cardiologist, said. “Our patients can continue to see the same doctors and staff who have provided cardiovascular care since 1968, just now at a new location. We are passion-driven and strive to provide affordable and accessible care. Lastly, we want our patients to know we will accept most major insurance companies.”

Chapman is among the 15 physicians who will comprise the Heart and Vascular Institute of Wisconsin team. The group specializes in all areas of heart and vascular disease, specifically coronary artery disease, arrhythmias, heart disease, heart failure, hypertension, and prevention, and are committed to individualized care plans for each patient. Additional services will include cardiac catheterization, ablation and pacemaker implantation and TAVR.

Current patients of these physicians can ask for their patient records to be transferred through their current provider via a release form. This form is also available at heartandvascularwi.com. New patients who wish to make an appointment can call 920.886.9380 between 8 a.m. – 5 p.m. to schedule a time. Current patients who have appointments scheduled after March 30, 2019 should call 920-886-9380 or visit heartandvascularwi.com to reaffirm or reschedule.
