What does this mean for me?

Our move to private practice means you can still see the same physicians you have grown to know and trust, just in a different location. The 15 physicians at the Heart and Vascular Institute of Wisconsin will continue the legacy of world-class comprehensive cardiovascular care that has been in place since this cardiology practice began in Appleton in 1968. With locations throughout northeastern Wisconsin, we are proud to stay at the forefront of medicine by providing a full array of advanced cardiac treatment and testing services in hospitals and office settings.

Where is your building located?

The Heart and Vascular Institute of Wisconsin located at 5045 W. Grande Market Dr., Appleton, WI 54913. We’re off College Avenue, near the Fox River Mall and Appleton International Airport.

Which doctor will provide my care?

All patients in our practice have a primary cardiologist who oversees their care.  However, we feel strongly that the optimal cardiovascular care requires a team approach.   In fact, we meet regularly to discuss complex and difficult patients to get the best thinking from all the expertise in our practice.   In addition, the large size of our practice allows us to recruit subspecialty physicians who are board certified experts in very unique aspects of cardiovascular care such as interventional cardiologists, electrophysiologists, vascular medicine, diagnostic imaging, etc.   We rely on our nurse practitioners and physician assistants to also provide direct patient care to patients.  The result is that over the course of your treatment you will likely encounter different providers while still being managed by your primary cardiologist.

What services will you offer?

Heart and Vascular Institute of Wisconsin is committed to providing a comprehensive array of services to our patients to treat coronary artery disease, arrhythmia, heart disease, heart failure, hypertension, prevention, and peripheral artery disease. At our facility, we offer a wide array of advanced cardiac diagnostic services as well as onsite procedures. Soon, we will also offer onsite cardiac catheterization, ablation and pacemaker implantation.

Will you accept my insurance?

We have obtained hundreds of insurance contracts, including Medicare and Medicaid as well as all the major insurance plans such as Aetna, Anthem/Blue Cross, Common Ground, Health Partners/Robin, Humana, Network, UnitedHealthcare and many others. Why did the physicians and staff decide to resign their employment from ThedaCare and return to an independent practice?

There are many aspects to the answer to that question.   It’s important to note that this cardiology group was founded in 1968 and has been an independent practice for all but 8 years in that time.   In summary, the physicians, and staff of the Heart and Vascular Institute of Wisconsin chose to return to being an independent physician practice so they could practice medicine and care for patients the way they were trained and the way they feel is best for patients. All at a lower cost.

What will happen to your outlying clinics?

Bringing care to local communities outside of Appleton has been part of the practice since its founding in 1968.   In addition to our main Appleton location, we have regional offices in Keshena, New London, Ripon, Shawano and Waupaca and are in the process of adding more locations.

What about your nurses and advance care providers?

We’re thrilled to say virtually every clinical employee was part of the practice while we were employed at ThedaCare has chosen to come and work with us at the Heart and Vascular Institute of Wisconsin. Not only will you recognize the physicians and providers, but also the APNPs, PAs, RNs, Mas, LPNs, diagnostic technicians and office staff. Together, these 100 employees bring more than 1000 years of combined experience in caring for cardiovascular patients.

What if I have to go to the hospital? Will you still see me there?

Yes! We will continue to offer the same level of service and care you’ve become accustomed to.  We currently care for patients at ThedaCare’s Appleton and Neenah hospitals as well as St. Elizabeth hospital.   We are in the process of expanding to other hospitals in addition to these three.

Will you have onsite laboratory testing or will I need to go somewhere else?

The Heart and Vascular Institute of Wisconsin has on-site laboratory testing. Unless the test is beyond our scope of capabilities, you will be able to have all your testing done right here.

Will you have access to my medical records?

For all patients who we previously cared for while we were part of ThedaCare, we have access to their current medical records in EPIC.   If you would like to transfer a copy of this information to be put into the electronic health record system we use at Heart and Vascular Institute, you will need to request their medical records be transferred to us. Federal law requires that a healthcare organization transfer your records. If you wish to do so, please download the Medical Records Request form. You will need to print this, fill it out, sign, then return it to us at: Heart and Vascular Institute of Wisconsin, 5045 W. Grande Market Dr., Appleton, WI 54913. We will then submit your request to have your records transferred to us.

What if I can’t print the medical records request form?

No worries. Simply call us at (920) 886-9380 and we would be happy to mail one to you. You can also stop by our office and fill one out.
