Insurance Carriers

Heart and Vascular Institute of Wisconsin is contracted with the following insurance providers, which includes several HMO and PPO plans, as well as Medicare and Medicaid. It’s important that you bring your current insurance card to each office visit, to assist us in properly filing your insurance claim.

If your health insurance plan requires authorization for a referral to Heart and Vascular Institute of Wisconsin, please first contact your primary care physician. If possible, we can assist you in obtaining your referral.

This list is for informational purposes only and is subject to change at any time. Some carriers have multiple provider networks and we may not participate in all the networks. You should always call your insurance company (the number is listed on your insurance card) to validate your coverage and whether or not we are in or out of network. If you do not see your insurance plan listed here that does not mean you cannot receive care from the Heart and Vascular Institute of Wisconsin.

There are many scenarios where this can happen:

• Oftentimes, an insurance company will approve individual treatments.
• We are happy to accept “out of network” reimbursement. Call your insurance company to see if we would be considered for “out of network” coverage.
• If you do not see your insurance plan on our list, call the Provider Contracting department at the insurance company and indicate that you would like us to be “in network”. Companies listen to their customers!
• Insurance companies often offer plans under a variety of names. Examples are Unitedhealthcare offering plans as “UMR” or Health Partners offering plans as “Robin”. If you do not see your plan on the list below it may be that one of our contracted plans under a different name.
