Initial Appointment For First Time Patients
A reminder system will either call, text, or email you before your appointment to confirm the date and time for your consultation visit. If you are unable to keep this appointment, please notify us as soon as possible by calling our office Mondays through Thursdays, 8:00 AM to 5:00 PM, or Fridays, 8:00 AM to 4:00 PM at (920) 886-9380
To make sure that you receive the time and attention required to provide optimal care, we ask that new patients arrive 15 minutes before their first appointment to complete the necessary registration process.
Please allow 1-2 hours for your consultation visit.
What to bring to your initial appointment:
- Insurance cards
- Photo ID
- Insurance co-payments
- Bottles of all current prescription and nonprescription Medications
- Completed New Patient forms
If you have been hospitalized or had cardiology testing at another facility, please have copies of the reports sent to our office before your initial visit.
Why it is critical to bring your current insurance card and valid photo ID
The two most important things to bring to your first doctor appointment, or any medical appointment, are:
- Your insurance plan ID card(s)
- A valid photo ID, like a driver’s license
Without a current insurance card, we cannot submit a claim to be covered by your insurance carrier. Without an insurance card, that means one of three things may happen:
- You will be billed for, and 100% responsible for, the full cost of your visit or treatment(s)
- You may be asked to pay for 100% of the cost of your visit or treatment(s) up front
- In some circumstances, we may cancel your visit or treatment(s) until you are able to provide us with your insurance information
Special Note to Medicare Patients Regarding Insurance Cards
New Medicare Cards Required January 1, 2020
To better protect the 60 million people enrolled in Medicare from identity theft, the government began issuing new, more secure Medicare cards last year via the mail. Wisconsin residents began to receive their new cards in June 2018.
Instead of identifying members by their Social Security number, the new cards use a computer-generated series of 11 letters and numbers that is unique to you. The cards also no longer include a person’s gender or signature. In addition, the new card is smaller, the size of a credit card, so it fits in your wallet more easily.
We are in a transition period where you can use either your old or new card. However, beginning January 1, 2020, we will no longer be able to accept and bill Medicare using the old cards and numbers.

If you need to replace your card because it’s damaged or lost, you have a couple of options:
- You can call 1-800-MEDICARE (1-800-633-4227 TTY 1-877-486-2048)
- Sign in to your gov account to print an official copy of your Medicare card. If you don’t have an account, visit MyMedicare.gov to create one.
